The Postsecondary Transition Advisor is part of the Game Plan: Future team, and supports Durham’s Citywide Summer Melt Campaign. The program is focused on helping high school seniors and recent grads navigate their next steps to college. This role manages targeted outreach, provides one-on-one guidance for students, coordinates with partners, and tracks student progress through fall 2026 to support a successful start to college.
